Smart Cremation, Smart Plan and smartcremation.com are registered trademarks of Smart Cremation, LLC.  In the State of California, the Smart Cremation services and merchandise are sold through Funeral Establishments owned by a sole proprietorship and using trademarks and service marks duly licensed from their respective companies.

A Special Note about Children

Children are not eligible to use our service and we ask that minors (under the age of 18) do not submit any personal information to us or use the service.

Required Information

To begin the process of making cremation arrangements online, you must provide your name, phone number and e-mail address to create a user account.

Vital Statistic Information

When you use smartcremation.com to complete Smart Cremation service arrangements, we will gather information about your loved one to complete a Death Certificate and other legal documents needed to fulfill your wishes for cremation.

Warning

The penalty for knowingly making a false statement on a Death Certificate can be 2-10 years in prison and a fine up to $10,000.00.

Unless noted, all information is required to fully complete these documents in accordance with the Department of Heath and Human Services and the Funeral Service Commission

Authorizing Legal Documents for Cremation

By digitally signing the cremation authorization form(s), you confirm the identity of the deceased person and the time and date of death; you have identified yourself as the authorizing agent and state the relationship between the authorizing agent and the deceased person; you certify that you are the authorizing agent that has the right to authorize the cremation of the deceased person and is not aware of any person with a superior or equal priority right. If another person has an equal priority right to authorize cremation, you state as the authorizing agent that you: have made all reasonable efforts but failed to contact that person and believes the person would not object to the cremation; you agree to indemnify and hold harmless the funeral establishment and the crematory establishment for any liability arising from performing the cremation without the person's authorization. We take your word that you are the legal next of kin. We have no way of confirming otherwise and are doing so in good faith. Our Use of "Cookies"

"Cookies" are small files of data that reside on your computer and allow us to recognize you as a Smart Cremation family if you return to the smartcremation.com website using the same computer and browser. We send a "session cookie" to your computer if and when you log in to your smartcremation.com account by entering your e-mail address and password. These cookies allow us to recognize you if you visit multiple pages in our site during the same session, so that you don't need to re-enter your password multiple times. Customer Service Correspondence

If you send us correspondence, including e-mails and faxes, we retain such information in the records of your account. We will also retain customer service correspondence and other correspondence from Smart Cremation to you. We retain these records in order to measure and improve our customer service, and to investigate potential fraud and violations of our User Agreement. We may, over time, delete these records if permitted by law.

Our Use and Disclosure of Information

Internal Uses

We collect, store and process your loved one's personal information on our servers located in Redmond, Washington. We use the information we collect about your loved one in order 1) to complete a Death Certificate and other required legal documents required for cremation, 2) to provide customer service, 3) to complete the cremation arrangements in accordance to your wishes and 4) to improve our products and services. We give access to individually identifiable information about our families only to those employees and agents who require it to fulfill the cremation wishes of the family. We provide protection against unauthorized disclosure of information you share with us. We limit associate access to our customer files only to those on a need-to-know case. We also educate these associates about their responsibility to protect your privacy, and we give them clear guidelines for adhering to our policies and standards. Our Contact with Smart Cremation Families

We communicate with our families on a regular basis via e-mail and telephone to provide requested services and to resolve customer issues. We use your e-mail address to confirm your opening of a smartcremation.com information file, to send information about important changes to our products and services, to send your password to you if it forgotten, and to send notices and other disclosures required by law. Generally, users cannot opt out of these communications, but they will be primarily informational in nature rather than promotional.

We also use your e-mail address to send you other types of communications that you can control, including any newsletters and satisfaction surveys. You can choose whether to receive some, all or none of these communications when you complete the registration process, or at any time thereafter, by logging in to your account on the smartcremation.com web site.

We use IP addresses, browser types and access times to analyze trends, administer the site, improve site performance and gather broad demographic information for aggregate use.
