Social Security
Call the Social Security Administration at 1-800-772-1213 or visit www.ssa.gov. Social Security benefits include a one-time benefit of $255, payable only to a surviving spouse or dependent children. Your Service Manager will assist with applying for Social Security benefits.

Public Assistance
Our Smart Plans are naturally set up as revocable. In cases where the Smart Plan holder is applying for assistance, it may be necessary for you to change the status of the plan to irrevocable. Simply call us at 1-800-700-2203 and we will assist in changing the plan to irrevocable.
